👉 Medical Bill Claim Rules for Delhi Jal Board (Step by Step Guide)

 मेडिकल फाइल सब्मिट करते समय निम्न बातों का ध्यान रखे और फाइलो के जल्दी निराकरण में हमारा सहयोग करे: 

1. क्लेम फॉर्म (फॉर्म C) में सभी बिलो की जानकारी अवश्य लिखे . 

2. सबसे पहले डॉक्टर की फीस का बिल उसके बाद उसी दिन का डॉक्टर का पर्चा और फिर उसी पर्चे की दवाइयों का बिल लगाऐ अगर साथ में मेडिकल जाँच भी है तो वे भी बिल सहित उसके साथ तुरंत बाद लगाऐं. 

3. अगर मेडिकल फाइल में एक से ज्यादा लोगो के बिल हैं तो बिलों को आपस में मिक्स ना करे, उन्हे क्रमश: लगाऐं.    पहले एक व्यक्ति के सभी बिलो को पूरा करें तब उसके बाद दूसरे व्यक्ति की जानकारी भरे. 

4. सभी बिल तारीख के घटते या बढ़ते क्रम में लगाऐं. 

5. एडमिट होने की दशा में B फॉर्म पर मरीज का फोटो और सभी बिल सम्बन्धित डॉक्टर से वेरिफ़ाई अवश्य कराये.

6. एडमिट होने की दशा में डिस्चार्ज समरी, फाइनल बिल ब्रेकअप, OT Notes / Progress Reports / Requisition Slip / Emergency Certificate हॉस्पिटल से लेकर लगा दे।

7. एडमिट होने की दशा में एडमिट होने के 3 दिन के अंदर और डिस्चार्ज होने के 7 दिन के अंदर दोनों बार डिस्पेन्सरी मे सूचना देना जरूरी हैं।

8. डिस्पेन्सरी बंद होने या किसी अन्य परिस्थिति में एडमिट होने पर 1916 या 9650291021 पर सूचित कर सकते है।

9. अगर मेडिकल एडवांस लिया हो तो सात दिन के अंदर मेड़िकल फ़ाइल जमा करा दे।

10. सभी मेडिकल बिल 6 महीने के अंदर जमा करने जरूरी है। उसके बाद मेडिकल बिल जमा नही होंगे. मेडिकल फाइल पर अपना मोबाइल नंबर जरूर लिखे और मेडिकल बुक की फोटो कॉपी, शाहदरा डिस्पेंसरी का रिकॉर्ड अवश्य लगा दे।

11. इंप्लांट्स जैसे कि लेंस, स्टंट आदि का बिल एवं कवर मेडिकल फ़ाइल के साथ जमा करे। महँगी दवाइयो के Cover/Invoice भी साथ में लगाए.
